If the stock levels in the SA Blood Bank are not replenished, our Doctors and hospitals will have a very difficult time saving lives.

So Medicare EMR would like to challenge all of you to take a moment and think about this situation we all find ourselves in. We all know what carnage the seasonal periods bring on the South African roads. So spare a thought for your loved ones and the same will be done for you. As it is said, "Do unto others as you want done unto you".

So Lead By Example and Others Will Follow! See You There!

South African Medical Journal, Vol 101, No 10 (2011) - Dr Alan MacMahon raises concerns re HPCSA

To the Editor: In a media release dated 30 August 2011, the
Professional Board for Emergency Care of the Health Professions
Council of South Africa states: ‘The HPCSA remains committed to the
discontinuation of the Basic Ambulance Assistants (BAAs), Ambulance
Emergency Assistants (AEAs) and paramedic Registers, thereby halting the current short courses offered.’ The closure of the registers is pending the promulgation of the relevant regulations by the Minister
of Health.

While the proposed discontinuation of the registers is to be welcomed,
the subsequent halting of the short courses is disturbing because it
displays a woeful lack of insight by the Professional Board into the
real world of operating ambulance services in South Africa.

As far back as 1982 I and other medical colleagues appealed to the
HPCSA to register advanced trained ambulance persons from the
provincial ambulance colleges in the Cape Province and Natal. The
reason for our request was that for the first time ambulance personnel
had been trained to administer specific drugs such as adrenaline and
atropine to patients.

We did not suggest registration of the basic or intermediate courses,
which even today form the backbone of trained ambulance personnel
delivering a service throughout the country.

It was predictable that to open registers for thousands of personnel
with short courses would lead to an unmanageable administrative
nightmare, especially when associated with collection of fees – and to
what purpose? Effectively the short courses are upgraded first aid
courses with the accent on ambulance operations in order to provide a
professional service to the public – certainly an asset and not a
hazard.

To discontinue these short courses as opposed to the registers will be
catastrophic to all the state-operated Emergency Ambulance Services,
which to a greater or lesser extent depend on these personnel. While
the registers should be discontinued, the courses should continue as
at present, subject to HPCSA accreditation of training facilities.

If all short courses were to be halted, what remains? (i) A 3-year
degree course offered only by the universities of technology, and (ii)
the 2-year ECT (Emergency Care Technikon) course.

Apart from the additional financial burden of effectively employing
only ambulance personnel with diplomas or degrees in the state health
service, it is tantamount to a waste of state resources, as no more
than 5% of patients require advanced life support.

The remaining courses are also more theory than practical, so even at
greater cost it is more unlikely that the public will be better
served!

R45 Claims another life...Please help us make the R45 Safer

Early yesterday morning Medicare EMR was dispatched to a serious MVA involving a truck that had ploughed into the mountain, trapping the four occupants. On duty medic Kurt Maske and his younger brother Kean, currently a first aider, were the first to arrive on scene. With operations hindered by limited communication, information had to be relayed via SAPS to Medicare EMR, Metro Rescue and ER24. In a combined operation between all attending, the AMS Medicopter and Metro Rescue 6 was dispatched.

Unfortunately the driver of the truck was killed in the impact as the truck, believed to have lost it breaks ploughed into the side of the mountain. The truck continued with momentum scrapping down the side of the mountain further trapping the other occupants in the crushed cab.

After a difficult 3hr rescue operation, AMS airlifted a critical patient for further treatment at a tertiary hospital. The other two occupants were lucky to get away with less serious injuries. The Franschhoek Pass remained closed for some time whilst the scene was cleared.

Thanks to all involved in preventing further loss of life and congratulations on a job well done. Condolences to the truck drivers family, you are in our thoughts and prayers.

Another Sad Day for South Africa - Several EMS injured in freak accident.

Our thoughts and prayers go out to the ER24 medics , SAPS officers and Fire officials that were injured in this mornings freak event in Bethlehem. They were attending to a MVA scene when a truck ploughed straight into them, with many sustaining critical injuries. From EMS in Cape Town you guys are in our prayers.

Into the skies I shall rise...you jealous yet?

I have flown around a few times in my life in planes and a few other Helicopters but nothing can begin to explain the feelings of excitement that I am feeling right now. I am really grateful that I was invited and not requiring this service myself.....

I'm gonna be in the ER 24 Medicopter tomorrow, next big challenge in my future will be to get a pilot's license because I know I'm gonna be hooked...this could be a problem... To the big blue skies, here I come.

Wow! What an amazing day it turned out to be, 1 x ECG/Defibrillator coming to the Franschhoek Valley! Thanks to all who made this possible!

Had a close shave today on the R45...Medicare EMR vs. Truck

Thanks to my constant alertness and evasive driving skills acquired over the years in the EMS was I able to avoid a head on with an oncoming truck who at that precise moment of approach had its right front wheel blow out.

This caused the truck to veer into my line forcing me into the shoulder only to have me watch the trucks rim break off and come rolling at a high speed towards me. I had unfortunately no where to go as to the left was a ditch and to the right was a truck. The rim hit the right front side of my response vehicle bursting my tyre on impact. At this stage I was already moments away from coming to a dead standstill.

I'm grateful for the rim not hitting a cats eye or a stone in the road, otherwise it would of become an air born projectile taking me and the windscreen out. This cat has more than 9 lives and its gonna take a lot more than that to scare me.

The point I would like to make is that when you are driving and something happens like this in front of you you need to the following to avoid causing an reaction vs. taking action. Confused? It's all about patience, if you hesitate, you have lost that opportunity, if you get that urge to take it then TAKE IT! When the truck veered into my lane, the immediate thing a average person would do is counter and veer into the opposite direction. While the truck driver is desperately trying to keep it under control and his immediate reaction is to get back onto his side of the road causing a head on.

So you see your reaction to his reaction(action being the tyre bursting) was pre-empted and you jumped the gun and made a fatal error. My advice is to be patient and rather watch what happens giving you the upper hand in being able to react on the actions of others rather than both reacting.

Kind of like the wise words once spoken - Be slow to anger.
